If you want to be put onto our housing list then you must follow these steps:
- First, read the 'Housing registration information pack'.
- Then, complete the 'Housing application form'. You must answer ALL questions.
- If you have specific medical needs then you must also complete the 'Health assessment questionnaire'. Please ensure your GP, consultant or a suitably qualified person completes and signs pages 8 and 9.
- And if you require sheltered accommodation, you must complete the 'Sheltered housing support questionnaire'.
Once you have completed all the forms, please send them to our Customer Services Team - firstname.lastname@example.org or 0800 5870211.
Please ensure that you answer all the questions in the application forms. Failure to do so will mean that your form will be returned and your application delayed. Incomplete forms cannot be accepted.
