This section contains application forms for registering for rented, shared ownership and sheltered housing with us. Other forms are also made available.

All forms are provided in a format which you can print out, fill in and return to us at The Garth. (You will need Adobe Acrobat reader to view these forms.)

• Housing registration form
• Housing application form
  This is the form you will need to fill in when applying for all types of accommodation including rented, shared ownership and sheltered housing
  
• Health self assessment form
  This form needs to be completed if you are applying to us for all types of housing and have a health condition or impairment which means that your current home is not suitable for you. If you are applying for sheltered housing you will also need to complete a housing support planning questionnaire which can be downloaded below.
  
• Sheltered housing support planning questionnaire for applicants
  This is the form you need to complete if you are applying for sheltered housing with us. The form will tell us about your support needs.
• Sheltered housing support planning questionaire for existing residents. This is the form you will need to complete if you are an existing JRHT resident.
  
• Garage registration form
  This is the form you need to complete to register for a garage in New Earswick.
  
• Anti-social behaviour policy and reporting form
  This is the form you need to complete if you wish to report an incident of anti-social behaviour to us.
